What private health insurance programs cover gastric bypass?

I've got group health and found out that only employer insurance programs through them include bariatric coverage. Does anyone know of a health insurance program that covers gastric bypass which I can buy on my own without needing to do so through an employer?

I live in NJ. I am 19 years old. I have had some problems in the past with my license. I have hit 2 Deer, had 2 2-Car Accidents, and have 3 Speeding Tickets. There was a period of 90 in which my license was suspended for these violations. I have been clean for a year now. Allstate has jacked my premium this year from $6K to $8K for 6 months. I wanted to change insurances. I tried to get Geico and Esurance but they won't insure me, even with my granddad on the policy as a primary. Esurance was going to cover me on Sunday along with my granddad, but today I called to give them my credit card # and someone different from the person I originally talked to on Sunday said that they cannot insure me. I have been without Insurance for a week now b/c I thought I was going to be with Esurance. What should I do now? How bad is it that I have been without insurance for 5 days? Should I go back to Allstate? Another thing I was thinking was dropping my insurance all together.

I am going to pick up my new car today IF this is a good deal. Its a black 2008 Toyota Corolla S. I am buying it, its a 72 month pay schedule of $387 a month. Insurance for it and our 2002 Santa Fe (both full coverage) is $157 a month. Do these prices sound about right? I want to know if I'm getting a good deal. Also, do new cars need to be maintinenced every 5,000 miles? Someone said that on another question of mine and I was wondering if thats true. Its almost time to renew my auto insurance policy, and I have a $1000 deductible. Ive thought about going down to a $500 deductible, because I recently had some minor damage to my car (a kid threw a rock at my window and it broke) and my insurance would not pay for it because the estimate was under $1000. Man, Im screwed. A few years ago I let my insurance lapse on my vehichle and didnt hand the license plates in. Failure to do so means an instant 150 dollar fine for the first month and 7$ a day afterwards. Ok, I was being a complete jackass at that time in my life and figured I would deal with it when the time comes. Well, its that time. I finally got my license back and went to buy a cheap used car. The dealer told me he cant get me plates for it cause I have a violation due to a fine for driving uninsured. The fine is off the charts, $4,455.00. Has anyone ever gotten the MVA to reduce this fee or waive it. I havent tried yet. They offer payment plans at 20% down and the rest must be paid within 2 years. I have been with Nationwide Insurance for 21 years. Your normal every day insurance can never come close to their rates and Nationwide has always had outstanding service. As we all know, money is lean these days so there are two companies I want to check out. I've just passed my driving test and was looking to get insured on my mums 1.6l Audi 80. So I went onto some comparison web sites and put my mum as main driver and me as the named driver and I was getting quotes of around 1900 (Fully comp). So I thought I would see what it would cost if my mum got the likes of a corsa or a fiesta or something. I changed the car details on the website and the quotes came out at 3000 + (Fully comp). I just don't understand how an Audi can be considerably cheaper than a corsa's and fiestas etc. Any explaination on why this is would be much appreciated. Also my mum really is the main driver as she will be using it for work and shopping. I will only be driving at weekends really.
